i have the opinion that this is not a Japanese problem. this term hikikomori might be a catch-all term for anyone who fits this pattern. i think some of these people are "pure" shut-ins who should be functional, but are able to isolate themselves because their family will fulfill their basic need. this happens in western countries, young people without jobs who gave up and live with their parents. it's just not talked about much, or maybe there is more opportunity to break out of these phases. and the rest may be non-functional, those who have mental health issues, like severe depression or spectrum of schizophrenia, and their family will also provide for them. in other countries, these non-fuctional people may be homeless or institutionalized, and wouldn't be called shut-ins. but i could be totally wrong. just my impression.
