I was listening to an interview of a medical doctor Astronaut who along with another crew member started to lose his near vision after an extended ISS stay.  Something to do with zero gravity was messing up the optic nerve and kinking it.  After returning it didn't correct itself, as of yet.  No one really knows what other types of never before seen medical conditions will arise on a long term space voyage, where the difficulty of providing treatment is compounded by limited equipment and zero g (eg. CPR is done by scissoring the patient across the cheast and compressing with the knees, getting visibility during surgery when the blood just kinda blobs everywhere and doesn't pool after suction).  He also explained that the evidence is mounting that dormant viruses (or some other name) that the body just carries around and never notice become stronger in zero g, while at the same time compromising the immune system, all within a small contained environment prime for spreading infection (snot globlets floating around...yuck)  So anyways, the first people to go will have to be some very brave people.
